Patients living within the practice area of BT36 and BT37 are welcome to register and our reception staff will need to see a signed medical card. If this is not available you will be asked to complete a registration form (GMS1) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team. This is known as an NPR (New Patient Registration). This needs to be carried out prior to any consultations with any of the GP's.

Medical treatment is available from the date of registration.  Please contact reception for further information.
